When Is the Best Time to See a Psychiatrist

For many people, the first conversation about anxiety, depression, memory changes, sleep problems, or changes in mood happens in the primary care doctor’s office.

And that is often exactly where the conversation should begin.

Collaborating with Your Primary Care Physician

Your primary care physician knows your medical history, medications, chronic conditions, and often your family and personal circumstances. But there are times when emotional, behavioral, or cognitive symptoms become complex enough that having a psychiatrist involved can provide an important additional level of expertise.

A psychiatric referral doesn’t necessarily mean that something is seriously wrong. It may simply mean that your doctor believes you could benefit from a physician who specializes specifically in diagnosing and treating mental health conditions.

When Should a Primary Care Doctor Consider a Psychiatric Referral?

A referral may be appropriate when symptoms are persistent, worsening, interfering with everyday life, or not responding adequately to initial treatment. The National Institute of Mental Health recommends seeking professional help when significant symptoms persist for two weeks or longer, particularly when they begin affecting sleep, concentration, appetite, relationships, work, or the ability to carry out normal activities.

Some of the concerns that may prompt a referral include:

• Depression that isn’t improving or keeps returning
• Persistent or worsening anxiety
• Panic attacks
• Significant changes in sleep or appetite
• Increasing irritability, agitation, or mood swings
• Difficulty concentrating or making decisions
• Loss of interest in activities a person once enjoyed
• Social withdrawal or isolation
• PTSD or unresolved trauma
• Obsessive thoughts or compulsive behaviors
• Concerns about alcohol or substance use
• Behavioral or personality changes
• Multiple psychiatric medications that need closer management
• Possible bipolar disorder, schizophrenia, or another complex psychiatric condition
• Memory loss, confusion, or cognitive changes in an older adult
• A psychiatrist can also be particularly valuable when physical health and emotional health begin overlapping. Certain medical conditions and medications can produce symptoms that resemble depression, anxiety, or cognitive decline, making collaboration between primary care and psychiatry especially important.

Older Adults May Show Different Warning Signs

Psychiatric care can become particularly important as we age.

Depression and anxiety in an older adult may not always look like obvious sadness or worry. Families may instead notice that Mom isn’t going out anymore, Dad isn’t sleeping, a normally independent person suddenly seems overwhelmed, or a loved one with memory impairment has become increasingly anxious, suspicious, agitated, or withdrawn.

Retirement, loss of a spouse, declining independence, chronic illness, caregiver stress, isolation, and cognitive changes can all affect emotional well-being.
